2. Create an Individual Inclusion Support Plan for this child in accordance with your service’s philosophy and policies. Be sure to include a copy of the service policy on inclusion with this plan. You may choose to use any template recommended by your teacher/trainer.
The plan must include the following information:
– Child’s first name;
– Age, additional need, interests, strengths, abilities & health status;
– Child’s background – e.g. culture, family structure, religion.
a) Describe two (2) strategies to support the child and the family when they first start using the service;
b) How you will develop and implement the plan in consultation with other professionals and the family;
c) How you will regularly exchange and share information with the family and appropriate community members (outside professionals) about the child’s needs and care strategies;
d) Research and identify the appropriate strategies for the child in order to meet his/her needs (within the scope of your professional role). Refer to EYLF when developing these. Be sure to include two (2) website links;
e) Explain how other children can be involved in supporting the child;
