Instructions
This is a 4-part assignment:
- Create a graphic of Leavitt’s Diamond for the program in which you are placed for your field internship. Use the template as discussed in class.
- Write a one-page description of the graphic elements referencing the readings for class.
- Prepare a one-page logic model and a one-page description of the logic model referencing the readings for class.
- Write accompanying paper of a total of 5 pages in APA format. The paper will briefly describe the program (one paragraph) and explain the ‘if-then’ connections in the logic model pointing out any discrepancies and making recommendations for programmatic changes.

Save your assignment as a Microsoft Word document (.doc or .docx – Mac users, please remember to append the “.doc” extension to the filename.) The name of the file should be your first initial and last name, followed by an underscore and the name of the assignment, and an underscore and the date.
An example is shown below:
