Practicum Onsite Visits

 

Summarize the key activities of your visits to your Practicum site (as appropriate), including with whom you met, what you did, and what you gained from the experience.

This week, I assisted in a Quality and Safety Council meeting, and a Practice Council meeting. The Quality Safety Council meeting started with moments of excellence which included announcements from the participants. The new B12 council member was also introduced (Mrs. Sawyer, Sade). Topics such as hospital acquired pressure ulcers (HAPU) report and the presentation of a new vacutainer for blood draws were discussed. In the area of Professional Growth Development, the quality data discussed how to disseminate the message to others consistently and focus on getting all nurses to truly understand the data presentations. As far as the Culture agenda, how to address the issue of CLABSI’s (Central lined-associated Blood Stream infection) and CAUTI’s (Catheter-associated Urinary Tract Infections) were discussed. Multiple people attended the meeting, but the key members who presented were Tim T., Jocelyn R., Jeff C., Jesse R., Lisa O., and Sade S., all nurses.

The Practice Council meeting also started with moments of excellence. The agenda items were as followed: “In the News” (Mercy  Magnet Mentor needed; 4 years commitment), Collaborative Practice which included infection prevention, education, pharmacy, laboratory, and technology report in which my preceptor discussed the upcoming new G-Tube assessment in Meditech, texting through responder 5 to PCD phones with canned and free text in the works; some part of Maternal Child Health have received a Health Stream 3-module introduction to the process of logging into the upcoming Epic system. Presenters were: April S., Lisa A., Susan H., Andrea L., Kim D., and Maria H.

 Write 3–5 pages in which you describe a nursing informatics best practices policy for effective and safe data use in a nursing practice setting or organization. 

 

Imagine an organization asks you to contribute to the development of an information system best-practices policy to help maintain patient safety and patient confidentiality in their practice setting. The organization wants you to create a document to help the group better understand why an informatics policy is needed and the practices that should be included in the policy.

A nursing informatics best-practices document can serve multiple purposes, such as a handout for new-hire orientation or as a background report given to a committee tasked with creating or updating a nursing informatics best-practices policy. Preparation

To expand your understanding of nursing informatics best practices within the industry, research the most current information about the topics of nursing best-practices documents and nursing informatics best practices.

 Locate 4–6 scholarly articles related to best practices designed to support positive patient outcomes related to the following areas: The meaning of secure data practices. Ethical standards needed to promote and support data security. Patient confidentiality. Regulatory requirements. Behaviors and skills nurse leaders need to guide the use of information technology and research for improved patient-care outcomes. Directions

Create a description of a nursing informatics best-practices policy document designed to define and encourage effective and safe data use in a practice setting or organization. Purpose Statement: Statement of why an organization would create the policy. Include any reasons and intent that supports the creation of an informatics best-practices policy. Best Practices Definitions and Descriptions: Definitions of secure practices, data security, and patient confidentiality. Ethical standards. Regulatory requirements. Implementation: Behaviors – describe the behaviors nurse leaders will need to demonstrate that will guide implementation of the policy. Skills required – describe the skills needed to ensure policy adherence.

Format your document using the professional format and style (corporate identity) used in your organization or practice setting. Note: If you are not currently working in an organizational setting, follow the formatting instructions below. Additional Requirements Written communication: Ensure written communication is free of errors that detract from the overall message. APA formatting: Format resources and citations according to current APA style and formatting guidelines. Number of resources: Cite a minimum of four peer-reviewed resources. Length: Submit 3–5 typed, double-spaced pages. Font and font size: Use Times New Roman, 12 point.

Provide a meaningful response to at least two of your peers’. In your responses to your peers, you might offer other examples from your own community.

First peer: The goals proposed were to reduce homicides, fire-arm related, bullying, child mistreatment, sexual violence, violence from coworkers, psychological abuse, self-harm, and the exposure of children to violence. I appreciate that considerable goals and thought were placed on those, other than the victim, affected by violence. Most often cycles of violence are perpetrated by intergeneration violence. That which was observed by children mimicked and displayed to their own children when they grow up. With children having greater access to violent movies, games, and television, violence is often downplayed and normalized amongst families and communities. Perpetuated violence escalates in their personal and professional lives. The most important goal in determining the effects of violence is an improvement in the reporting and coding of violence for unified measurement and tracking.

second peer: The goals identified by Healthy People 2020 that are related to reducing violence is progressed to achieve these objectives can save thousands of lives, reduce the suffering of victims and their families, and decrease the financial cost to the law enforcement and healthcare systems. The role that nurses can and do play in violence prevention is critical and extends beyond just caring for victims to also include preventing violence before it happens. Bullying can have serious health, physical, and psychological effects on the student who bullies, the student who is bullied, or the student who both bullies and is bullied. School nurses can educate students and staff and advocate for student support. Also, include examining school rules and using behavior management techniques and social-emotional learning in the classroom and throughout the school to detect and provide consequences for bullying.

  

To prepare:

· Select a client whom you have observed or counseled at your practicum site. ( EX 23 years old white male with depression and suicide ideations).

· Review pages 137–142 of the Wheeler text and the Hernandez Family Genogram video in this week’s Learning Resources. Reflect on elements of writing a Comprehensive Client Assessment and creating a genogram for the client you selected.

Part 1: Comprehensive Client Family Assessment

With this client in mind, address the following in a Comprehensive Client Assessment (without violating HIPAA regulations):

· History or present illness

· Past psychiatric history

· Medical history

· Family psychiatric history

Part 2: Family Genogram

Prepare a genogram for the client you selected. The genogram should extend back by at least three generations (great grandparents, grandparents, and parents).
