Practicum Onsite Visits

 

Summarize the key activities of your visits to your Practicum site (as appropriate), including with whom you met, what you did, and what you gained from the experience.

This week, I assisted in a Quality and Safety Council meeting, and a Practice Council meeting. The Quality Safety Council meeting started with moments of excellence which included announcements from the participants. The new B12 council member was also introduced (Mrs. Sawyer, Sade). Topics such as hospital acquired pressure ulcers (HAPU) report and the presentation of a new vacutainer for blood draws were discussed. In the area of Professional Growth Development, the quality data discussed how to disseminate the message to others consistently and focus on getting all nurses to truly understand the data presentations. As far as the Culture agenda, how to address the issue of CLABSI’s (Central lined-associated Blood Stream infection) and CAUTI’s (Catheter-associated Urinary Tract Infections) were discussed. Multiple people attended the meeting, but the key members who presented were Tim T., Jocelyn R., Jeff C., Jesse R., Lisa O., and Sade S., all nurses.

The Practice Council meeting also started with moments of excellence. The agenda items were as followed: “In the News” (Mercy  Magnet Mentor needed; 4 years commitment), Collaborative Practice which included infection prevention, education, pharmacy, laboratory, and technology report in which my preceptor discussed the upcoming new G-Tube assessment in Meditech, texting through responder 5 to PCD phones with canned and free text in the works; some part of Maternal Child Health have received a Health Stream 3-module introduction to the process of logging into the upcoming Epic system. Presenters were: April S., Lisa A., Susan H., Andrea L., Kim D., and Maria H.

First peer: The goals proposed were to reduce homicides, fire-arm related, bullying, child mistreatment, sexual violence, violence from coworkers, psychological abuse, self-harm, and the exposure of children to violence. I appreciate that considerable goals and thought were placed on those, other than the victim, affected by violence. Most often cycles of violence are perpetrated by intergeneration violence. That which was observed by children mimicked and displayed to their own children when they grow up. With children having greater access to violent movies, games, and television, violence is often downplayed and normalized amongst families and communities. Perpetuated violence escalates in their personal and professional lives. The most important goal in determining the effects of violence is an improvement in the reporting and coding of violence for unified measurement and tracking.

He has been diagnosed with hypertension for the last twelve years and Type 2 diabetes for the same period of time. His current blood pressure is 162/90, with a pulse of 76. His body mass index (BMI) is 32. He is currently taking Maxzide 37.5/25 mg every morning. This is the only antihypertensive medication he has taken.

For this question, focus on the treatment of hypertension. Discuss the influences of his age, gender, and ethnicity on hypertensive medications. On the basis of an analysis of those factors, provide one option for improvement of his blood pressure and provide a clear and specific justification for that choice. Be sure to include dosage and scheduling. Include highlights of patient teaching and/or recommendations for any lifestyle changes. Her current cholesterol is 240 mg/dL, HDL is 41 mg/dL, LDL is 163 mg/dL, and triglycerides are 183 mg/dL. Her blood pressure today is 174/94, and her pulse is 90.

Review the medications she is taking for hypertension and dyslipidemia. Evaluate the efficacy of these medications. Review them in terms of her age, gender, and ethnicity. Suggest any changes you would recommend, with clear justification for those choices. For medications, include dosages and schedules. DQ 1

New health care delivery models are being presented to accommodate the shift in health care objectives. Many of these models are community-based and focused on improving quality outcomes, population health, and reducing readmissions to acute care settings. Describe a new health care delivery model and discuss whether or not it is sustainable.
