Assignment: Mrs. R., an 87- year- old patient, has a past history that includes coronary artery disease, a previous stroke, and advanced Alzheimer’s disease. Ten days ago, Mrs. R. was hospitalized for aspiration pneumonia and has been ventilator dependent since being admitted to the intensive care unit in a small rural hospital. Family members visit daily and have repeatedly voiced their concern to the nursing staff about the continued ventilator support that Mrs. R. is receiving, most notably the fact that Mrs. R. would never have wanted such care. They also note that Mrs. R. has not recognized them in past months and that they plan to visit less in future days, but can be contacted should any change in Mrs. R.’ s condition occur. Her primary physician has practiced in this community for multiple years; he is well known for his reluctance to discontinue any type of life support for any patient. When questioned, Dr. G.’ s consistent response is, if this were his frail 92- year- old mother, he would prescribe the very same treatment for her. Dr. G. has now requested that the nurses talk to the family about moving Mrs. R. to a major medical center, where she can receive more advanced care, including vigorous rehabilitation and physical therapy, so that she may eventually return to a long- term nursing care facility. How might the nurses in this scenario respond to the physician’s request? How would this scenario begin to cause moral distress among the nursing staff, and what are the positive actions that the nurses might begin to take to prevent moral distress? Read Exercise 4.1 (Guido, p. 44) and apply the MORAL model to resolve the dilemma. Read the case study presented at the end of Chapter 3. Examine the above scenario from the perspective of health care policy. How would you begin to evaluate the need for the policy and the possible support or lack of support for the policy from your peers, nursing management, and others who might be affected by the policy? Do the 10 questions outlined by Malone (as listed in Guido, p. 50) assist in this process? Draft a proposed policy to address the issue you have identified.
  What compelling right does this case address? Whose rights should take precedence? Does a child (here, a competent 14-year old) have the right to determine what will happen to him? Should he ethically have this right? How would you have decided the outcome if his disease state had not intervened?

 The difference between research utilization and evidence-based practice

            Research utilization is basically conducted to process the research and to obtain more data for the research that was conducted earlier. It is a method of incorporating and applying research based intervention practically. Results of a certain study are implemented into a practice that are not related to the study, this is referred to as research utilization. It focuses specifically on applying scientific research findings in nursing practices. Research utilization is used to alter the impact on the practice of nursing. It includes application of opinions from the experts and patients’ reports, which can be applied in the treatment process (Schaffer, Sandau & Diedrick, 2014).

Evidence based practice involves the practice and performance of care and the treatment process which has already been implemented successfully or given positive results. Evidence based practice enhances the nursing practices in healthcare and ensures the quality of the care given to the patients is improved. Hence, implementing evidence based research in nursing practices helps to increase knowledge and elevate the nursing care standards. Evidence based practice implements the best current evidences to come up with better decisions on health care. It is very useful to researches related to nursing since it supports the hypothesis of the nursing research and scientific literature, which helps in the clinical trials (LoBiondo-Wood & Haber, 2017).

Evidence based practice is research utilization that employs other considerations specific to the problem that is being solved. Evidence based practice uses the present evidences to make better decisions regarding the patients while research utilization aims to put in practice the implementation of the research study.

Ergonomics

Employee safety extends to their workspaces. For HIM Professionals, this often involves an office, cubicle, or other space that contains a desk, computer, monitor, filing cabinets, and so on. You may be seated at that desk for extended periods, and this can lead to discomfort. In extreme cases it can lead to injuries in your back, neck, eyes, or wrists (carpel tunnel).

“Ergonomics” is the engineering and organization of objects and spaces to minimize physical discomfort and potential for accident or injury. It is very important. As an HIM professional, you will be called on to educate new employees. This should always include ergonomic training as well. You may also be called upon to improve workspaces with ergonomics in mind.

Look around the area where you are taking this course. Identify three areas that you feel are ergonomically correct and three things that could be improved. ( note:3 thing correct: computer high its right and easy to adjusting, maintain good position, chair can be adjust in the right level of the eyes. 3 thing need to change is the keyboard not in the right place, the ac in the room was way to high, can get sick because outside way to hot, chair cannot adjust the lower back in the right place,).
