Case Study – The Elderly

Scenario:

As a community health nurse, you are working in an outpatient clinic in a community that serves mainly elderly clients.

This year, you noted that 85 percent of the visits resulting in hospitalization resulted from bronchitis, asthma, pneumonia, and influenza as compared to last year. At that time; the visits weredue to colds, sore throats, influenza, bronchitis, and pneumonia, in order of frequency.

Maria Perez, a 73-year-old Cuban American woman has been referred for community health nursing services following her discharge from the hospital. She smokes two packs of cigarettes per day. She was recently hospitalized as a result of a severe shortness of breath due to an asthma attack. The primary care provider prescribed respiratory treatments using Albuterol via jet nebulizer every 4-6 hours as needed. Mrs. Perez lives alone and has three cats to take care of.

Instructions: Read the scenario above and answer the following questions:

What are the biophysical, psychological, physical environmental, sociocultural, behavioral, and health system factors influencing Mrs. Perez’s health? Describe at least three primary, secondary, and tertiary prevention strategies that would be appropriate in resolving Mrs. Perez’s health problems. Why would they be appropriate? What activities related to health promotion and disease prevention would you plan for the late fall? How would you evaluate your nursing interventions? What criteria would you use to evaluate care?

1. According to the Gallup polling organization, in 2001, 45 percent of Americans believed abortion is morally wrong, but by 2009, the percentagehad risen to 56 percent. During the same period, the percentage of people who believe that it is morally acceptable to have a baby outside of marriage rose from 45 to 51 percent. However, the percentage who believe gay and lesbian relationships are morally wrong dropped from 53 to 47 percent1. 

Suppose that two of your friends are discussing the meaning of these statistics. One says, “The statistics mean that gay relationships and having children out of the wedlock used to be immoral but now they are moral.  On the other hand, abortion used to be moral but now is immoral.”  The other responds, “That’s ridiculous.  If an action was wrong a decade ago, it is still wrong today; and if it wasn’t wrong then, it isn’t wrong now.”

12. A 16-year old girl visits a birth control clinic and asks to be put on the pill.  Because she is a minor, the clinic doctor who writes the prescription for her notifies her parents of the action.  Possibly a majority of Americans would approve of the doctor’s action.  Is the action ethical?

13. In some parts of the country, a majority of the citizens presumably believe that it is morally acceptable to send people to jail for smoking marijuana. Is that view correct?

 Compare and contrast two change theories, and determine which theory makes the most sense for implementing your specific EBP project. Why? Has your mentor used either theory, and to what result? 

In nursing practice, accurate identification and application of research is essential to achieving successful outcomes. Being able to articulate the information and successfully summarize relevant peer-reviewed articles in a scholarly fashion helps to support the student’s ability and confidence to further develop and synthesize the progressively more complex assignments that constitute the components of the course change proposal capstone project.

For this assignment, the student will provide a synopsis of eight peer-reviewed articles from nursing journals using an evaluation table that determines the level and strength of evidence for each of the eight articles. The articles should be current within the last 5 years and closely relate to the PICOT statement developed earlier in this course. The articles may include quantitative research, descriptive analyses, longitudinal studies, or meta-analysis articles. A systematic review may be used to provide background information for the purpose or problem identified in the proposed capstone project. Use the “Literature Evaluation Table” resource to complete this assignment.

While APA style is not required for the body of this assignment, solid academic writing is expected, and in-text citations and references should be presented using APA documentation guidelines, which can be found in the APA Style Guide, located in the Student Success Center.
