                                                                   Global Nursing Issues

            Two of the largest challenges related to providing adequate health care in underdeveloped countries are of the Inversed and impoverished types.  These “care” dynamics are interconnected by similar principals; people with the most consume the most and those who have the least get the least.  In my opinion, these concepts mimic Darwinism, which supports, “survival of the fittest”. An interesting off-shoot of Darwin’s Theory, is the claim that consanguineous marriages result in genetic anomalies.  It stands to reason the results of inadequate provisions of healthcare and the survival of only those who have the means to pay will survive.  This environment fosters the narrow circle from which one can reproduce.  This can create a situation where a populous with a decreased immune system and an inability to fight off infection requires extra measures that steal from those who do not. As a nurse who has worked in clinical research I can attest to the outcomes of incest. The results of smaller populations can be catastrophic and devastating for the children who result.  My strategy for preventing situations such as this is to provide nursing education to communities where the cultural or environmental behavior is commonly practiced and part of the accepted culture. There are “countries” within our own country that are third world by virtue of the definition of how healthcare is distributed.  

Nurses have the ability to influence policy and to have an effect on decisions that impact quality of life and universal access to care. Becoming influential in policy work is a process that requires commitment to developing skills and acquiring knowledge about activities that lead to change. It involves making choices that involve participating in policy formulation by learning how policies of interest to you are developed. Identifying influential people who are participating in policy development can help establish connections.  Also, professional organizations such as ANA, AARP, NAMI, and many others are useful professional resources. Investigating the health care agendas of our local legislators, especially if elections are up for renewal, by volunteering and participating in policy meetings or other processes where you learn, will ultimately have an impact on policy change nationally and globally.

MS Excel is a great help with calculations which are completed using formulas. Remember, by typing an equal sign (=) into a cell, you are preparing Microsoft Excel to do a formula calculation. Refresh your memory on the use of formulas from this week’s assigned reading and the online lectures. For Sheet 2, think of a situation at home, work, or even a hobby or sport for which you could use a calculation and create a formula to solve a problem.
Open a spreadsheet in MS Excel and create your formula. Make sure you label components of your spreadsheet. Scenario
Tom and Sally were trying to get a handle on their budget. They heard that MS Excel could help them with that.  Tom is going to school and has cut back on his hours. Sally has finished her associate’s degree and is working full time. They want to look at the last three months of their income and expenses to see where they can make changes.
Part I – The Spreadsheet
First, in an Excel Spreadsheet, create a budget to show Tom and Sally’s income, expenses, and money leftover each month (Net). Create formulas in your spreadsheet for your calculations using the following data:
Tom brought home $1,000 a month for January, February, and March. Sally brought home $1,900, $2,000, and $1,975 respectively for those three months. They paid $1,000 for rent and utilities each month. They paid $88 for insurance each month. They paid $60 for cell phones and $60 for Internet/TV each month. They spent $600, $750 (due to a big birthday bash), and $500 on food respectively. Their car payment and gas came to $225, $250, and $300 respectively. Entertainment and gifts were $75, $100, and $45 respectively. They paid a little extra on their credit card $150, $125, and $100 respectively. They allocated $200 each month for personal grooming and health care. They put $100 each month into savings. Miscellaneous Expenses came to $200, $100, and $400 respectively. A sample budget layout is included below as an example of one way to lay out a budget.

Part II – The Chart
Once you have worked out the budget spreadsheet, use the tabs at the bottom of the page to open another sheet. Here you will create a chart similar to the example below using the tools you learned about in the lectures and the textbook. On the new sheet:
Create a column chart to visually represent their monthly expenses. If you have a challenge getting the chart on this sheet, research how to move your chart using your reading or Help (F1). You might also try right clicking on the chart. Add a descriptive title to the chart.
Part III – Changing Values
Copy everything from Sheet 1 (the budget calculations) and paste it into a new worksheet. Change values for Sally’s March income to $200 more. Let’s say that they didn’t use as much heat in March and their utilities were $75 less (you could use a formula to figure out $75 less but you don’t have to). See how the totals change automatically if you used formulas correctly. If the values did not change, check your formulas and try them again. Seek help if you can’t figure this out. Rename each of your three spreadsheets with descriptive names. By the assigned due date, submit this budget to the appropriate submission folder. In the message box, share how the process of creating this went for you, including any challenges, successes, or insights.

My program is Nurse Educator and to do research on anything to do with this is a little difficult as there is not much information on it. I decided to do two types of searches based on how I might go with my master’s degree. The first search was nurse educators and what came up was an article on how to become a better nurse educator to nursing students. The most interesting statement made in this article hit close to home for me. “Teaching student nurses’ critical thinking and the ability to apply clinical reasoning skills remains a challenge in nursing education” (Wyngaarden, 2018). Being a nursing student is stressful but having to think outside the box but quickly can be even more difficult at times. Teachers are having a hard time connecting with students because not only is there a huge age gap between the student population but so many different learning barriers and language barriers they are trying to work around. 

The second search I did was nurse educators to patients. I research this as well because I have also thought about being an educator to patients to have one-to-one conversations about their illness. I feel as though patients might listen and understand better if they are working with someone one-on-one rather than a different nurse/doctor every other day while in the hospital. While I was working for Shands, they had a diabetic nurse educator who would not only do education with the patients during their hospital stay, but she would go to the clinic and speak with patients. Once a month she would hold a newly diagnosed diabetes class for patients and then once a week she would hold classes on how to continue to control your diabetes. She would charge $5 for the continuing education class. The money that she would make from this would go to buying supplies to hand out to patients who did not have any or could not afford it. 

For both searches, I used the Walden Library and found it to be the easiest way to look for my information. The Walden Library has a variety of articles/books/online journals to choose from. I will always recommend using the Walden Library for any type of research as this provides information from all over as well as the information is current.

Discussion: Drug Treatments for HIV/AIDS

While HIV/AIDS is still currently incurable, the prognosis for patients with this infectious disease has improved due to advancements in drug treatments. Consider the case of Kristy Aney. Kristy was diagnosed with HIV in 1992 and was told she would survive, at most, 10 more years. Despite unfavorable odds, Kristy is still alive 20 years later. Since her diagnosis, she has witnessed tremendous improvements in HIV/AIDS treatments which have helped patients live longer with fewer side effects. While she acknowledges that these drug treatments have kept her alive, she fears that improvements in drug therapy have led to more people becoming complacent about the disease (Idaho Statesmen, 2012). In fact, the number of people living with HIV/AIDS in the United States is higher than it has ever been (CDC, 2012). This poses the question: Is there a relationship between drug advancements, societal complacency, and infection?

To prepare:

· Review Chapter 49 of the Arcangelo and Peterson text, as well as the Montaner et al (2014) articles in the Learning Resources.

· Reflect on whether or not the prevalence of HIV cases might be attributed to increased complacency due to more advanced drug treatment options for HIV/AIDS.

· Consider how health care professionals can help to change perceptions and make people more aware of the realities of the disease.

· Think about strategies to educate HIV positive patients on medication adherence, as well as safe practices to reduce the risk of infecting others.

With these thoughts in mind:

· An explanation of whether or not you think the prevalence of HIV cases might be attributed to increased complacency due to more advanced drug treatment options.
