List of Hazards to be used.

  • Wheel falls off of vehicle
  • Christmas tree catches on fire
  • Turkey burns in oven
  • Vehicle does not start
  • Ceiling fan falls from ceiling

1) Create a FTA for 1 of the hazards listed. ( at least 3 levels – Do not need to assign probability )

2) Use any of the methods from lecture to analyze another hazard listed.( PHA, FTA, FMEA, Bow Tie )
