Consent Form for Interview
|
- If your participants are adults, use the consent form.
- If your participants are under age 12, use the assent form under age 12.
- If your participants are over ag 12, use the assent form over age 12.
- Use the template as it is – Only change the red-colored sentences. Do not mix with other forms.
- Information in the consent form should be specific enough, so that participants can fully understand what will happen during the interview process.

Interview Protocol
|
- Your interview questions should be matched with your research questions.
- Your interview questions also should be matched with the interview type (unstructured interview, semi-structured interview, or structured interview).
- Your interview questions also should be matched with your research design.
- For general qual, ethnography, case study, and grounded theory research, you will prepare approximately 10 – 15 main questions. Include possible probing questions as much/little as you see fit.
- For phenomenological study and narrative inquiry, you will prepare approximately 2-3 main questions. Include at least 10 possible probing questions.
- Include a way to follow up with them as well as them with you.
- It should have three sections; (1) introduction including your research topic and research purpose/questions, (2) actual interview questions, and (3) closing paragraph.
- Use “Interview Protocol Template” as a guideline.
- Once you finish your interview, you cannot go back. Think hard to generate the best set of interview questions, which can help you to answer your research questions.
