Explain where you will make changes and why.
What information did you gather and analyse when identifying the changes that need to be made?
From what sources did it come?
Outline the rationale for the change, the change objectives, the procedures to be followed and the expected benefits of the change.
How will you manage, resource and implement the change?
What reporting, monitoring and review protocols will you follow?
Develop, document and present a completed change management project plan.

Answer all of the questions and submit the plan and your answers. Answers might be verbally presented, but the presentation will need to be sufficiently detailed and presentation notes must be submitted.
