orientation for new healthcare quality surveyor

I have attached 2 documents, an onboarding from HR at work and an orientation form.

I need a written orientation form which includes a general orientation and one that is more role specific.

The role is a surveyor for healthcare quality, used for accreditation such as Joint Commission International (JCI) or any accreditation under the International Society for Quality in Healthcare
