Does Not Meet Expectations1 APA format and style are not evident Hi All Here is an explanation of what is expected in the assignment this week. This is a signature assignment, which means that no part of the assignment can be changed, it is evaluating some key components of the program. I have put a few definitions to try to clarify what is being asked for in the assignment. I have also put verbiage of what is being looked for in each component You use your case study from week 2 as a reference. You do not have to do any more than just a bare overview of the family, if you feel you need to include it in your paper. You also do not have to do an overview of the community-you did all this in week 2. This paper is NOT so much about the family, but general information like: 1.What is the role of the community nurse. Who are community partners and what is their importance to the nurse?[evaluate] 2.What are social determinants of health? What impact does culture, access to resources etc have on families in your identified community?[evaluate] 3.Epidemiologic data-you need two levels of data, like local to state, or state to US. Data is how you demonstrate there is a need. This helps support what you focus on in your LHI. 4.For the problem you are picking for your Leading Health Indicator (LHI)-what is the local data and the state data? Or what is the state data and the US data?[analyze] 5.What is the biggest issue faced by your case family? Why?[analyze] Apply that issue to the community to develop a community nursing diagnosis. Write the diagnosis following this format: Increased risk of _____[disability or disease] Among ____[community or population] Related to _____[etiology statement] As documented in _____[health indicator] Now-what health partnerships in the community could the family benefit by accessing? [explain] (here is the link to the CDC leading health indicators: https://www.healthypeople.gov/2020/vindicators ) 1.What nursing intervention would you do for the biggest problem the case family has? Why? [explain] 1.Now-based on the issue you picked as your LHI-create a plan for how nurses can advocate within the community for sustained change that includes goals, planned action, needed resources and a timeframe for evaluation. Remember this is COMMUNITY focused, not a plan for an individual [create a plan] Include References!
