You will work in groups and prepare a report including the following componets given in the table below. According to Shi (2008), factor analysis is a statistical technique that involves exploring the relationships between variables, determining if related variables can be explained by a smaller number of underlying factors or constructs, and reducing the number of variables in the analysis, if appropriate (p. 386). Factor analysis could be applied to any number of health services research topics in which current knowledge in incomplete (Shi, 2008, p. 386). For example, factor analysis would be used in the assessment of validity in the method of construct validity where the validity can be measured using factor analysis, which would indicate whether the clustering of items chosen to be part of a particular measure have been grouped together appropriately (Shi, 2008, pg. 297). Another example includes the measurement of depression and anxiety. In order to create an appropriate screening tool that distinguishes between depression and anxiety, a researcher could perform a factor analysis on a pilot screening tool. He or she would screen a sample of patients using the tool, and then perform a factor analysis to see how well (or how poorly) the different types of questions hang together (Shi, 2008, pg. 386).

Multilevel analysis techniques are useful when data exist at several different levels (Shi, 2008, pg. 387). If data are nested, or if a study has several different units of analysis, multilevel analysis is a valuable tool. For instance, multilevel analysis could be used in: a study comparing quality improvement achievements of departments within several different hospitals, a longitudinal study in which several distinct observations are made on individuals over time, or a meta-analysis in which subjects are nested within different studies (Shi, 2008, pg. 387).

the people who are working for caregivers, is service providers to the patients in several dimensions. they need to understand the needs of patients, treated them, serve them with care, affection and love. can you imagine how a 2-year-old kid behaves, the same way these people to care of the patients. the benefits of these people are, they get more satisfaction than any other. because they are serving people who are not able to take care of themselves. god gives us birth, but rebirth only gifted by doctors only. hence, caregivers are doing the job perfectly. the drawbacks of this job are, many of the patients and their relative people does not know how these people are working. many of the people assumes that, they are paying, then why don’t they do better than this.

it is all about quality of service the caregivers offer to patients. these caregivers spend most of the time with patients only. they do not have much time to spend with their family and for personal obligations.

these people must offer a little high pay than existing level of pays. as i already said that, these people psychologically bear much tension and personally, and heartfully serve to the people. assume that you are repairing a car, how do you deal it. you do not ask anything the car. just you proceed based on the problem, and the car never expects any return from you. but a patient, whose health is in trouble, expects many things from you. you are there to cure the medical problem along with psychological problems of the patient. the patient expects something from you, he asks, gives suggestions, raise his voice, create more hurdles, may not react properly and so on. but at the end it is your talent, to make him perfect and cure from the problem. so, it is better to offer some high pays to the caregivers in the industry.

when these people get some good income, a greater number of people takes this as employment opportunity and more kind of concentration will be taken on patients. they caretakers never treat the patients as patients, they always treat them like their birth child only. they only they can serve as caregivers, otherwise they cannot survive in the profession.

Should Mary’s family members authorize the double hip replacement?

No, I do not believe Mary’s family members should of authorize the double hip replacement. This is not due so much to her advance age since there are many very active 87 years old seniors that would benefit from this.  It is due to her condition deteriorated physical condition.  She is in a wheelchair most of the time due to her arthritis, has a heart condition plus she suffers from diabetes. These factors make her is a high-risk patient due to medical complications. Just the fact that she has diabetes, she is more likely to suffer from infection after surgery, slower healing than normal plus heart problems.  Also, the arthritis will not help when it comes to the recovery part of the post-surgery making physical therapy extremely difficult.  Furthermore, the heart condition will play a factor when she is put in anesthesia.

What factors should be considered?

The factors that should be consider are first her age. The older a patient is the higher the risk for any operation. Then there is the diabetes which brings complications of its own to anyone that suffers from it. Some of them are slower healing, infections and heart problems. We also know that she has a heart problem that would compound the issue when it comes to general anesthesia. Finally, there are the last two factors that go hand in hand that will play a factor in her post operation. There is her arthritis along with her mental condition. These two conditions will make it very difficult to due physical therapy to insure a proper recovery due to the pain she will be in. Also, her mental condition that makes her withdraw from the real world will compound that problem with the recovery.

Who else, if anyone, should be considered?

This is a somewhat a hard question to answer. The first person that should be considered would be Mary since she is the one who is having the surgery. The problem here would be her mental status which can make her response very hard to assess how she feels. The seconded person I would consider would be the surgeon that will be doing the procedure. S/he would be the best qualified in this scenario to say what would be that outcome of the surgery and what are the risk in having it or not having it.

What are the implications of that decision on the parties involved: Mary, her children, the facility staff?

The implications of the decisions would be as follow. For Mary who is the one most affected would be the continuation of the deterioration of her health.  If she does not have the surgery, her hips will get worse as the time goes by. On the other hand, if she does have it. She is a very high-risk patient. For her children would be the ramifications of either decision since they are in a non-winning situation.  They will have to consider the pros and cons to better their mother’s health. Since she can’t make it on her own. The last would be the staff who will have to care for the patient in worsening conditions of try to give acute care along with prolong care for Mary’s recovery.
