In this assignment, you will be completing a health assessment on an older adult. To complete this assignment, do the following:Perform a health history on an older adult. Students who do not work in an acute setting may “practice” these skills with a patient, community member, neighbor, friend, colleague, or loved one. (If an older individual is not available, you may choose a younger individual).Complete a physical examination of the client using the “Health History and Examination” assignment resource. Use the “Functional Health Pattern Assessment” resource as a guideline to assist you in completing the template.Document findings of complete physical examination in Situation-Background-Assessment-Recommendation (SBAR) format. Refer to the sample SBAR Template located on the National Nurse Leadership Council website at https://www.ihs.gov/nnlc/includes/themes/newihstheme/display_objects/documents/resources/SBARTEMPLATE.pdf as a guide.Document the findings of the physical examination in the assessment worksheet.Using the “Health History and Examination” assignment resource, provide the physical examination findings summary with planned interventions for the client. Include any community services in the interventions.

In 1-2 Paragraphs give an answer

The U.S. Presidential Administration of Barack Obama passed the controversial Patient Protection and Affordable Care Act (ACA). The Affordable Care Act (ACA) is one of the many influences that impact the financial structure of healthcare today. It addresses three balancing goals within the United States’ health care system: 1.) access, 2.) costs, and 3.) quality. Based on your perception of the Act’s purpose, do you think all three goals can be effectively reached? For example, can you increase access and reduce costs while increasing quality? What are other financial influences that relate to ACA or controversial health care issues? Explain your point-of-view and compare your findings to at least one other classmate’s.

Integrating Evidence-Based Practice

Write a 1000-1500 word essay addressing each of the following points/questions. Be sure to completely answer all the questions for each bullet point. There should be two main sections, one for each bullet below. Separate each section in your paper with a clear heading that allows your professor to know which bullet you are addressing in that section of your paper. Support your ideas with (2) sources (1 outside source and the textbook) using citations in your essay. Make sure to cite using the APA writing style for the essay. Part 1:

Describe the eight steps to integrating evidence-based practice into the clinical environment. What barriers might you face in implementing a new practice to address your research topic ( clinical practice guideline towards the prevention of  chronic neck pain among adults. therapy over medication) Describe strategies that could be used to increase success including overcoming barriers. 

Part 2: 

Describe six sources of internal evidence that could be used in providing data to demonstrate improvement in outcomes.
