As revenue generators, NPs must be aware of how their work contributes to the overall revenue of the clinical practice. You see 20 patients per day on average and take call every third weekend. According to Buppert (2011), an NP who sees 15 patients per day at $56 per patient visit, on average, brings in $840 per day. Allowing 1 week off for continuing education, 1 week off for illness, and 4 weeks off for vacation, this NP will bring in $193,200 a year, potentially. However, not all bills are paid. With a 90% collection rate—a reasonable collection rate for an efficient practice—this NP actually will bring in $173,880 per year. An NP who sees 24 patients per day will bring in $1344 per day, or $309,120 per year in accounts receivable. With a 90% collection rate, this NP will bring $278,208 to the practice (Buppert, 2011).

Establishing a salary can be a challenge for NPs. Deducting 40% of the NP’s gross generated income for overhead expenses (rent, benefits, continuing education, supplies, malpractice, lab expenses, and depreciation of equipment) leaves $104,280 for the 15-patient-per-day NP and $166,925 for the 24-patient-per-day NP. Further deducting 15% of that figure to pay a physician for consultation services leaves $88,638 in salary for the 15-patient-per-day NP and $141,887 in salary for the 24-patient-per-day NP. Deducting 10% for employer profit leaves $79,775 in salary for the 15-patient-per-day NP and $127,699 for the 24-patient-per-day NP (Buppert, 2011).

What salary would you propose for the contract renewal? How does your salary proposal fit in with the community standard for an NP in a similar practice? Use logical reasoning, and provide evidence based rationales for your decisions. Keep in mind that your negotiation terms and conditions must be within the legal scope of practice for an ANP.

Scenario: A 12-year-old girl is brought to the emergency room (ER) by her mother with a complaint of shortness of breath, wheezing, tachypnea, tachycardia, and a nonproductive cough. The mother states they had just come from a fall festival, where the entire family enjoyed a hale ride. They also participated in the corn maze challenge. The symptoms began shortly after they left the festival , but got better a couple of hours after they returned home. The symptoms began again about six hours later and seemed to be worse. “As a concept, need is not only difficult to define but, once defined, difficult to measure” (Kettner, Moroney, & Martin, 2017, p. 63). Likewise, it can be challenging to define and measure assets—yet it is equally important to do so during this phase of program planning. This week’s Learning Resources provide foundational information and guidance for conducting needs assessments, which entails discerning needs that must be addressed in light of the assets that are present.

As you return to Assignment 1, the attention turns to conducting a needs assessment for the problem you have identified. What data collection strategies could be most useful? What issues would likely be encountered? How would you address them?

To prepare for this week’s section of Assignment 1: Continue      (from the Discussion) to: Identify       a viable approach for conducting a needs assessment for your problem. Identify       method(s) of data collection. Assess       potential challenges and propose alternatives/solutions.

Chapter 1 notes the importance of looking not just at needs but also the assets associated with the target population and environment; that is, assessing what needs to be addressed given the assets that are present.

This report offers a guide for the use of the logic model in program planning and outcome-oriented evaluation for nonprofit projects.

Chapter 3, “Data Collection Strategies for Needs Assessments and Evaluations”

In Chapter 3, the authors examine data collection, a key element of needs assessments. They also indicate that it is wise to plan ahead and consider data collection strategies for evaluation at the same time. Chapter 2, “Paint a Picture of Your Target Population:      Assessing Assets and Problems” 

This chapter presents valuable information for identifying your target population during program planning. Note: Some of the information in this chapter extends into needs assessment, which will be addressed in Week 4.

The authors describe various theories, noting that theories are not universally applicable to every program.

Obesity is not exclusive to adults. Each day, more and more children are finding themselves at risk for overweight and obesity. Childhood obesity often accompanies many of the obesity-related conditions adults affected by obesity often experience, such as type 2 diabetes, hypertension, sleep apnea and more. Recent data shows that up to 80 percent of children affected by obesity will continue to be affected by obesity into adulthood (Lobstein, et all, 2015). Obesity impacts children in a variety of ways. First and foremost, a child’s health is impacted as they have now opened themselves up to a wide variety of health issues – issues that most of us didn’t experience until middle-age. In addition to health implications, there’s also one other area that children face which can be very serious – weight bias and bullying. Kids impacted by obesity often find themselves the target of bullying. This bullying can take place in the classroom, in your neighborhood and even in your own home. It is very important to recognize this type of behavior and address it quickly (Lobstein, et all, 2015). The Obesity Action Coalition (OAC), a nonprofit dedicated to educating and advocating for those affected by obesity, provides valuable resources on weight bullying.

Over the last decades, food has become more affordable to larger numbers of people as the price of food has decreased substantially relative to income and the concept of ‘food’ has changed from a means of nourishment to a marker of lifestyle and a source of pleasure. Clearly, increases in physical activity are not likely to offset an energy rich, poor nutritive diet. It takes between 1–2 hours of extremely vigorous activity to counteract a single large-sized (i.e., >=785 kcal) children’s meal at a fast food restaurant (Lobstein, et all, 2015). Frequent consumption of such a diet can hardly be counteracted by the average child or adult.

It has been hypothesized that a steady decline in physical activity among all age groups has heavily contributed to rising rates of obesity all around the world. Physical activity strongly influenced weight gain in a study of monozygotic twins (Rivera, et all, 2014). Numerous studies have shown that sedentary behaviors like watching television and playing computer games are associated with increased prevalence of obesity. Furthermore, parents report that they prefer having their children watch television at home rather than play outside unattended because parents are then able to complete their chores while keeping an eye on their children (Rivera, et all, 2014). In addition, increased proportions of children who are being driven to school and low participation rates in sports and physical education, particularly among adolescent girls, are also associated with increased obesity prevalence.

Almost all public health researchers and clinicians agree that prevention could be the key strategy for controlling the current epidemic of obesity. Prevention may include primary prevention of overweight or obesity itself, secondary prevention or avoidance of weight regains following weight loss, and prevention of further weight increases in obese individuals unable to lose weight (Rivera, et all, 2014). Until now, most approaches have focused on changing the behavior of individuals on diet and exercise and it seems that these strategies have had little impact on the growing increase of the obesity epidemic.
