Discuss a clinical problem or issue in your area of practice that would be appropriate for an EBP project and formulate a researchable question using PICOT format. The discussion should not be too long about 2-3 paragraphs.

The initial posting (IP) and the response to peer  posting each (RTP) require one reference from an English titled, peer reviewed NURSING journal less than 5 y/o. The references must be from different journal articles.

IMPORTANT: The articles posted in each unit are to facilitate your understanding of the topic and help you meet the identified learning objectives. They are not to be used as references for any of your course work. 

However, FOR THIS WEEK ONLY:  I want you to use the articles in Unit 1 as your sources.

Eveluating diet

 

Research a diet currently being used by the public. It can be a commercial diet such as Atkins, Weight Watchers; a regional diet such as Mediterranean; or one that focuses on types of food such as vegan.

Identify the name of the diet and its purpose (weight maintenance, long term health, etc.). Explain its key points in terms of nutrition (focus on protein, carbohydrates, etc.). Also discuss the diet plan and the diet’s strengths and weaknesses. Summarize your paper by evaluating whether you believe the diet is one that is healthy as compared to dietary guidelines.
The writing assignment should be no more than 2-3 pages and APA Editorial Format must be used for citations and references used.

 How to Write the Introduction 1. Write the hook. The hook must include a metaphor word. What is a metaphor? A metaphor is a comparison between two unlike things. For example, I’m drowning in homework. 2. Use the S.O.A.P.S.Tone acronym to guide you through the middle of the introduction paragraph. 3. Thesis/Claim In one sentence, use strong language to express the purpose of your argument. Use an implicit thesis. Since the first paragraph is a counterargument, start the thesis sentence with that information. Example Thesis: Though not all people agree, it is evident that nursing facilities must treat LPNs better, because these employees are the most needed. How to Write the Conclusion 1. Write the telegraphic sentence. Show your concluding thought in a short sentence. Thus technique helps capture the readers’ attention. 2. Point-by-point method Look at your body paragraphs- counterargument, argument 1, argument 2, and argument 3- and write one sentence for each body paragraph. These sentences should be in a positive tone and they should remind the reader of the paragraph’s purpose. 3. Restate the thesis/claim. Using the explicit thesis statement structure, restate the thesis/claim. 4. Wrap the hook. Refer back to the metaphor word you used in the hook (introduction). Write the final sentence of the essay using a word related to the metaphor word you opened the essay with. • The conclusion should have 7 sentences. • There is no need to cite sources in the introduction and conclusion paragraphs.

Mrs. Smith is an 85 year female with a history of Hypertension, diabetes and osteoarthritis. Her daughter who cares for her reports that she became suddenly confused beginning last night. She recognizes her daughter but has been unable to remember her grandchildren’s names. She continuously asks her daughter “What are those kids doing in our house?” Her mother was in and out of bed all night and slept very little. Her daughter does say that her mother seemed better this morning, but on the way to your office she became more confused again. Review of systems is significant for a dry cough for the past few days. She has not had a fever. Her appetite has been poor. Her exam is normal except for some fine crackles in her left lung base. Mrs. Smith’s daughter says, “My dad had Alzheimer’s and now it looks like I am going to have to go through that with my mother”

1) What are the possibilities for change in Mrs. Smith’s current mental status?

The treatment of anaphylactic shock varies depending on a patient’s physiological response to the alteration. Immediate medical intervention and emergency room visits are vital for some patients, while others can be treated through basic outpatient care.

Consider the January 2012 report of a 6-year-old girl who went to her school nurse complaining of hives and shortness of breath. Since the school did not have any medication under her name to use for treatment and was not equipped to handle her condition, she was sent to an emergency room where she was pronounced dead. This situation has raised numerous questions about the progression of allergic reactions, how to treat students with severe allergies, how to treat students who develop allergic reactions for the first time, and the availability of epinephrine in schools. If you were the nurse at the girl’s school, how would you have handled the situation? How do you know when it is appropriate to treat patients yourself and when to refer them to emergency care?

To Prepare

· Review “Anaphylactic Shock” in Chapter 24 of the Huether and McCance text, “Distributive Shock” in Chapter 10 of the McPhee and Hammer text, and the Jacobsen and Gratton article in the Learning Resources.

· Identify the multisystem physiologic progression that occurs in anaphylactic shock. Think about how these multisystem events can occur in a very short period of time.

· Consider when you should refer patients to emergency care versus treating as an outpatient.

· Select two patient factors different from the one you selected in this week’s first Discussion: genetics, gender, ethnicity, age, or behavior. Reflect on how the factors you selected might impact the process of anaphylactic shock.

Post an explanation of the physiological progression that occurs in anaphylactic shock. Then, describe the circumstances under which you would refer patients for emergency care versus treating as an outpatient. Finally, explain how the patient factors you selected might impact the process of anaphylactic shock.
