Case Scenario

Mrs. J. is a 63-year-old woman who has a history of hypertension, chronic heart failure, and sleep apnea. She has been smoking two packs of cigarettes a day for 40 years and has refused to quit. Three days ago, she had an onset of flu with fever, pharyngitis, and malaise. She has not taken her antihypertensive medications or her medications to control her heart failure for 4 days. Today, she has been admitted to the hospital ICU with acute decompensated heart failure.

Subjective Data

  1. Is very anxious and asks whether she is going to die.

  3. Says her heart feels like it is “running away.”
  4. Reports that she is so exhausted she cannot eat or drink by herself.

Objective Data

  2. Vital signs: T 37.6 C, HR 118 and irregular, RR 34, BP 90/58
  3. Cardiovascular: Distant S1, S2, S3 present; PMI at sixth ICS and faint; all peripheral pulses are 1+; bilateral jugular vein distention; initial cardiac monitoring indicates a ventricular rate of 132 and atrial fibrillation
  4. Respiratory: Pulmonary crackles; decreased breath sounds right lower lobe; coughing frothy blood-tinged sputum; SpO2 82%
  5. Gastrointestinal: BS present: hepatomegaly 4 cm below costal margin

Critical Thinking Questions

What nursing interventions are appropriate for Mrs. J. at the time of her admission? Drug therapy is started for Mrs. J. to control her symptoms. What is the rationale for the administration of each of the following medications?

  1. IV furosemide (Lasix)
  2. Enalapril (Vasotec)
  3. Metoprolol (Lopressor)
  4. IV morphine sulphate (Morphine)

Describe four cardiovascular conditions that may lead to heart failure and what can be done in the form of medical/nursing interventions to prevent the development of heart failure in each condition.

Taking into consideration the fact that most mature adults take at least six prescription medications, discuss four nursing interventions that can help prevent problems caused by multiple drug interactions in older patients. Provide rationale for each of the interventions you recommend.

Essay Instructions

A critique is a formal evaluation of a text, and one of the most common forms of critique in literature is the review. In a review, you – as the reviewer – get to evaluate the text. For your first essay, choose one of the short stories I’ve assigned to review. Your 35 page review should include quotations from the text wherein you will cite your sources according to MLA guidelines. Here are some guidelines for writing your review:

Introduce What You Are Evaluating

• Include the title and author.

• Be clear about what you are evaluating

Set up Your Review with a Summary

• Your summary is to be used as a reference point for your discussion.

• Be sure that your summary is balanced with adequate analysis. This is not a book report.

Put the Piece into Context

• What type of work is it?

• When was it written?

Analyze the Text

• Note how the work is structured.

• Look at the individual elements: plot, character, dialogue.

• Determine the purpose of the work.

Include Your Reasoned Opinion: This Is Your Evaluation

• Did the work achieve its purpose?

• What is your response to the selection and why?

• Agree or disagree with the presentation of information (whether or not it achieved its purpose).

• Base your agreement or disagreement on evidence.

End with a Balanced Conclusion

• Recap the pros and cons of the piece.

• Give your overall reaction.

Crisis-Counseling

Sometimes clients come into the counseling session with an immediate crisis. They need an immediate solution to a problem. Counseling in these situations needs to be focused and goal oriented.

Jamila has been working with her counselor on learning to better manage her 9-year-old son. Last night, her son was hit by a car and is in the hospital. This has created a number of problems:

  1. She has to arrange childcare for her daughter so she can stay with her son in the hospital.
  2. He will need some special care when he gets home because he will be in a wheelchair.
  3. She is fearful that she will lose her job as a teacher’s aide in the local public school.

Write a 500-750-word paper addressing the following:

  1. How would you prioritize Jamila’s concerns in the counseling session? What would you address first?
  2. What steps would you take in this session to make sure that Jamila has the resources she needs right now?
  3. How would you help Jamila develop a plan to manage the current crisis?

Include at least three scholarly references in your paper.

Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is required.

story-and-obsevations

  1. Write a short paragraph synopsis of the play. (between 2 -5 sentences is plenty)
  2. Write 2-3 paragraphs about the production values (lighting, sound, set and costume design, acting skills, overall production etc.) of the show. Offer constructive observations about what you liked and/or disliked about the production – make sure you give details of why you have your opinion e.g. I enjoyed the lighting effects especially during the opening of the production, when two spot lights drew my attention immediately to the two actors on stage. This helped me to know where to turn my focus, and also gave the characters a sense of isolation, setting up their overall struggle through the play.
