For your final Portfolio Project, you will write a paper detailing a risk management plan based on the following case study scenario. Your goal is to identify areas of risk and healthcare/medical error, and to safeguard future patients from having their safety compromised in a manner similar to what occurred in this case study. Your risk management plan must include:

  • A root-cause analysis
  • At least three recommendation(s) for improvement
  • Two recommendations for how this situation could be avoided
  • Identification of all roles in your analysis
  • Quality, risk, and performance improvement diagrams and charts to support your analysis (e.g., a fishbone or other visual form of root-cause analysis, Pareto chart, tables, etc.)
  • Commentary that relates the case broadly to what you have learned throughout the course and describes the roles played by quality management and regulation to prevent unfortunate occurrences such as the case described

Your paper should meet the following requirements:

  • Be 10-12 pages in length, not including the cover or reference pages.
  • Be formatted according to the CSU-Global Guide to Writing and APA Requirements.
  • Provide support for your statements with in-text citations from a minimum of eight (8) scholarly references—four (4) of these references must be from outside sources and four (4) may be from course readings, lectures, and textbooks.   • Utilize headings to organize the content in your work.

The case is as follows:

You are the Chief Risk Management Officer for a large metropolitan not-for-profit teaching hospital. You have been assigned this case by the Chief Executive Officer due to the potentially sensitive nature of the situation and the CEO’s personal interest in disadvantaged and ethnic minority population groups. Your goal is to do a thorough risk analysis and create a risk-management plan as part of the overall quality management program for this facility based on the following case scenario.

A female patient, age 25 years, presents in the emergency room with pain in the right side of her lower abdomen. Her last name is Jonesky. She has been ill and vomiting for two days. The patient is triaged and asked to take a seat in the emergency room waiting area until she can be seen. Additionally, the patient speaks only Russian, and her husband, who is the only family member with her, speaks limited English, with Russian being his primary language.

Several hours have passed and the patient is still sitting in the emergency room, in extreme pain. The husband approaches the emergency room desk to try to communicate that his wife is worsening and in extreme pain. After several minutes of discourse, the patient is taken back into an examination room to be seen.

She is diagnosed with appendicitis and requires removal of her appendix. She is sent to a holding area outside of the operating room where she will wait to be taken into surgery. Upon her arrival, a nurse asks her name and due to the fact that the patient only speaks Russian, her name, “Jonesky” sounds very much like “Jones” to the nurse. Then anesthesiologist arrives and interviews the patient. Simultaneously, another emergency patient is brought into the holding area of the operating room. Her name is Samantha Jones. Samantha has fallen down icy steps and has broken her left ankle, which now needs to be repaired. This is a very busy night for the operating room. Additional staff members, who have already worked a full eight-hour shift, are called in to attend to this second case.

Both patients are interviewed and taken into ORs for their procedures. Since it is late, both surgeons do the required time-out procedure, but not as judiciously as they would have during the day. Staff members in attendance for both cases implicitly go along with their surgeon’s order and surgery following a time-out procedure that was not complete. As the first surgical procedure begins, the surgeon makes an incision into Mrs. Jonesky’s ankle and there is no sign of any broken bone. He realizes there is a problem. Additionally, in the second OR, the surgeon operating on Samantha Jones makes an incision and finds no sign of appendicitis. Both surgeons operated on wrong sites due to incorrect identification of the patient.

Healthcare organizations face many unprecedented challenges and opportunities. Diversity is one of the major challenges that continues to overwhelm management teams. Gender, ethnicity, and race are most noticeably lacking in top leadership roles in healthcare. There is a documented need for diverse leadership teams with transformational leadership skills to successfully lead organizations. Remediation of this gap is critical to the advancement of healthcare delivery and systems. Senior management must consider the deficiency and develop a strategic process to invoke change.

Suppose you are the Executive Director of the hypothetical organization Better Health Today, LLC. The board of directors has informed you that the organization has missed out on major federal funding opportunities over the past three to five (3-5) years. The grant reviewer’s feedback describes the organization’s lack of diversity and community support to promote social change. The board of directors has asked you to review the issues noted in the reviewer’s feedback and provide recommendations to improve the reputation of the company.

Write a five to six (5-6) page paper in which you:

  1. Based on your knowledge of leadership theory, specify two (2) key competencies needed to improve community relations. Provide support for your rationale.
  2. Examine the impact that the lack of diversity has on the community meeting national public health initiatives. Analyze two (2) of the major challenges the organization faces in improving diversity.

    A company that has undoubtedly grown and expanded by diversification strategy and business models is that of Procter and Gamble. It is the world class leader in its industry, this company is easily identified and commonly known for the manufacture and distribution of their household goods. Its beauty and grooming business line generate more than 70% of its revenue, thus, making it their most dominant business line of all times.

    Through vertical integration P&G has managed to make an easy and smooth transition into other related markets and enable them to not only exploit and expand those markets but add value and competitive advantage to an already known value chain. In the process, they have learned that adding value through diversification should not only rely on brand name products such as Tide, Crest, Charming and Dawn products but through the creation of a better customer service experiences and customer excellence practices.

    These changes have translated into making sure that customers are served “in different ways but all with a focus on making peoples’ lives a bit easier.” (P&G) According to Forbes article, “Here’s How P&G’s Brand Consolidation Program Could Backfire and Push Its Valuation Below $60” written by the Trefis Team, P&G developed a program to consolidate all higher revenue generating brands in an effort to purge and rid of all other product lines that were depleting their growth potential. The biggest threat to this new concept is the death “of diversification due to the brand consolidation program” (Trefis Team, 2015) but firm’s CEO cites that “as a result, P&G will be able to transform itself into a far leaner organization with minimal loss of revenues.” (Trefis Team, 2015)

In one to two paragraphs, compare and contrast the source you described in your initial post with the one described by your peer. Are the two sources’ theses or arguments compatible?     1.)The secondary source that I chose to use is Scientists and the Decision to Bomb Japan by David H. Frisch. The lens that the article is using is Economic as it discusses the National and International Business Strategies on if they should drop the atomic bomb military style without a demonstration first to notify Japan of the attack to test the performance of it. The article also talks about finding the area size for a demonstration capable of keeping the damage minimal. It also talks about determining if the use of the bomb will be worth the lives of civilians in order to end the war. General Groves states from his book, “Now It Can Be Told” “To enable us to assess accurately the effects of the bomb, the targets should not have been previously damaged by air raids. It was also desirable that the first target be of such size that the damage would be confined within it, so that we could more definitely determine the power of the bomb.”

    2.)When thinking about the type of lenses this author possibly chose, my mind falls on the covering all three lenses. The way Frisch speaks it seems obvious that he is from a small town and doesn’t really understand the thought process of those higher up and how the got to that conclusion of just bombing them instead of doing a test run. “Although my special experiences were quite limited, Los Alamos was a rather intimate community, so I hope to have in reasonable perspective the feelings of many who were there” (D. Frisch, 1970). I would use this same quote for the social aspect of his argument as well. He lives in a very intimate town where most people are like minded. The government lenses are him talking about how he basically doesn’t understand what the exact reasoning was for the two bombings to go off so close together. The way he phrased it made me think he was talking partial conspiracy. “Perhaps a demonstration would have been followed by too short a pause before a very destructive bombing to allow the Japanese government time to absorb the facts, as Hiroshima seems to me to have been followed too closely by Nagasaki” (D. Frisch, 1970). This author I used for my own projects for this course because his view seems to make sense to me and what I feel about the bombings. I also think that made it easy to see what type of lens he may be looking out of.
