Kindly find attached “Burn Care case”. This is the “quantitative analysis” case.

A few thoughts on Burn Care…

  • You are a VERY-WELL recompensed consultant, hired by Mr. Warden the hospital CEO, to develop and analyze alternative strategies regarding the situation. Mr. Warden is the target audience for the report and presentation.
  • As specified in the Case, “The corporation has a perfect Management Information System. I am providing you with the necessary data to solve the problems. Your job is to reduce operating expenses by $500,000 without reducing the quality of care or services to patients.
  • You must also avert the nurses’ strike and maintain their long-term job satisfaction. You can implement any change, use any strategy to reduce your expenses. If your strategy involves capital expenditure, then it must be paid for with interest within three years by the additional savings in operating expenses.”
  • For the Burn Care case your report should be the typical comprehensive “Capstone” report’ 15-pages single spaced report and these 15 pages not including charts, tables, or figures.

Let’s Review the Case

Job One –> The hospital was given a goal of reducing operating expenses by $500,000 for the burn care unit. (p. 2), based on 1994 hourly rates.

This is Larry’s favorite case, and there are a lot of ways to approach finding the reduction. But, you must not provide sub-standard patient care, and you want to prevent a nursing strike. Also, remember, you should be concerned about your continued ability to attract and retain qualified nurses – unless, that is, you want to get all of them from the agency pool (but are agency nurses as good, what about quality and continuity??).

A couple of more observations and/or subtleties I am concerned you might miss: Be careful

  • On page 2, the case says –> In May 1993 Mr. Adams, the regional vice-president of the corporation and an accountant by training, wrote a letter to Mr. Warden, the CEO, that he should discontinue the Baylor Plan as it was costing about $800,000 more in nursing payroll. Obviously, IF Mr. Adams is correct, finding $500,000 in savings will be easy… right??? Or, is Mr. Adams just a stupid budget weenie in administration who doesn’t understand or even have a clue?? So, never ever say this in your solution.
  • Remember, the “Budgeted Daily Staffing Pattern” (p. 4) is based on an assumption of 30 ADC, but what happens when ADC is higher/lower? How does the pattern change? And, remember, the budget model is based on the staffing standard of 22 HPPD.
  • Pay attention to this paragraph –> The hospital’s budgeted nurse-staffing standard in 1996 was 22 hours per patient per day (HPPD). The staff nurses on the unit, however, generally complained that the staffing levels were not sufficient to provide quality care to the burn patients. In December 1996, the hospital implemented Medicus patient acuity
  • Make sure you understand the work sampling data on page 5 🙂
  • Make sure you understand the “Census and Acuity Data by Pay-Period” on page 3 🙂
  • From page 6 –> Here we are assuming that only the regular staff makes overtime under the Baylor Plan. The Flex staff is generally a part-time staff and works 8- or 12-hour shifts without overtime but they receive benefits. Agency nurses do not earn overtime or benefits from the hospital.
  • Obviously, regular employees are paid OT at 1.5x for hours worked over 40 in a week. If you suggest something besides the Baylor staffing model, make sure you are explicit about how that schedule will work for “regular” employees – 24×7 staffing is very, very hard and is typically not easily divisible by 40.
  • What about sick leave, annual leave, etc. for regular employees. How does that fit in?

Description

Compare a larger mainstream grocery store chain (Select one: Giant, Harris Teeter, Safeway, Shopper’s Food Warehouse, Trader Joe’s, Whole Foods, Walmart, Target) with an international food market listed on this Google Map (Select one: Super A Market, Food Star Market, Bestway Supermercado, H Mart, Grand Mart, Good Fortune).  Keep in mind that this project requires that you make site visits to several locations.  Spend at least 45 minutes at each location recording field notes.

Procedure

Step #1. We begin by describing a grocery store. During class you will diagram a store, and  make predictions regarding what you will find during your visit.

Step #2. Visit both stores, spending at least ½ hour taking field notes in each location. In addition  to recording general observations, you will use a list of questions to guide your  observations. For example:

  • Before entering the store, describe relevant aspects of the area/neighborhood

  • Describe the overall store layout (lighting, advertisements, decorations, background music).

  • How are the aisles arranged? Are any aisles labeled for “ethnic” items? If so, how are these aisles labeled? Within the aisles, how are products organized?

  • What kind of products are being sold? Where did these products come from? For example – are there fresh vegetables and fruits?  If so, where was this produce grown?

  • Are products described as ‘organic’?

  • Who frequents this store? What can you tell based on the type of products that are being sold? Also consider the people you see in the store (shoppers & clerks).

  •  Does the store appear to accept forms of public assistance as payment (EBT, WIC, SNAP, etc.)?

 You should take detailed field notes, which you will turn in with your final paper.  In your observations, be on the lookout for both similarities and differences.  Also, please take a picture at the store of an item you do not anticipate the other store to have. Record the price of one item that both stores have (milk, bread, or other common items, etc).

Step #3. Place your observations in social context. Learn more about the neighborhoods  surrounding these two food markets. Who lives in these neighborhoods? Has this  changed over the last two decades? Use the links provided as your starting point.

Written Report (Approximately 1000-1500 words):Take a step back to reflect on what we can learn sociologically about our local community from this comparison of different food markets in our region. Describe what you observed in the summary section: how are the two food markets similar and how are they different? Using any relevant sociological concepts that we have covered in class this far, interpret your observations.

CASE MANAGEMENT VIGNETTE- “THE GONZALES FAMILY”

The Gonzales family is a Mexican-American family consisting of the
mother Maria age 36, step-father Henry age 46, the oldest daughter Mary
age 16, the oldest son James age 13, a set of twins Sam and Jordon ages
7, and a newborn son Henry Jr. age 10 months. The older children Mary,
James, and the twins have a different father, who is currently in prison
for armed robbery. Both Maria and Henry are unemployed, although Henry
reported working almost full-time in manual labor jobs, until about 3
months ago when he was seriously hurt at work. He stated it is painful
to walk for long periods at a time and his back always seemed to hurt.
Henry is in the country illegally, which has made it difficult to seek
full-time employment other than manual labor jobs. They recently
received an eviction notice from the landlord for the three-bedroom
apartment they live in, due to three months of missed payments. They
have 30 days to be out of their apartment, or pay the back rent. The
parents report they have no medical insurance and currently live off of
Maria’s welfare check each month. However, they have had to sell a
portion of their food stamps each month to have cash to pay the utility
bills. Maria reports having very little food left in the house, and it
is only the 15th of the month. Maria reported struggling with depression
for most of her life and at times turning to drugs when things “get too
bad”. The family initially came into therapy because James was recently
suspended from school due to bad behavior. Maria stated that James has
always struggled in school and she is concerned he may have some
learning problems.

Glad to hear you are coming to visit me. I normally have a very busy week so I prepare food on Sundays. I cook roasted chicken, curry chicken or goat, and fried fish. These meat goes easy with salad throughout the day and when I come home at 7, I normally cook the rice or dumpling.
