My strengths are the ability to listen and empathize with my patients, no matter their backgrounds. But I have learned through the years not to engage in spiritual talks unless the person brings it up first. This may be a weakness on my part, in terms of witnessing to others, but I have learned that those who are not seeking will often reject what is said to them while trying to convince others of their views.

I am blessed to have well educated people in my life, and a family I trust to make decisions for me, should anything happen. I feel that in the event I end up incapacitated with an uncertainty to my recovery, it would be the lives of my family members burdened by mine, and they should have the final say in what happens to me – or if I even get treated. Yes, I believe doctors have the obligation to treat patients who are likely to make a recovery, but ultimately I believe that patients who are oriented should be able to accept or refuse any treatment. In terms of family members choosing refusal of care for someone who would most likely recover, I believe that the hospital should petition the court for guardianship.

Referrals

Patient care hinges in part on adequate and timely information exchange between treating providers. Referral and reply letters are common means by which doctors and nurse practitioners exchange information pertinent to patient care. Ensuring that letters meet the needs of letter recipients saves time for clinicians and patients, reduces unnecessary repetition of diagnostic investigations, and helps to avoid patient dissatisfaction and loss of confidence in medical practitioners.

As a Nurse Practitioner (NP) you will need to know the difference between a consultation and a referral for treatment, when ordering and when carrying out consultations or referrals.

Consultations

A consultation is a request for opinion or advice, so that the requestor can manage the patient. A consultation is billed under one of the consultation codes listed in Physicians’ Current Procedural Terminology (CPT) (99241-99245 for outpatient of office consultations). If the NP is the consultant, the NP should document the request for a consultation, the reason for the consult, and the NP’s evaluation and recommendations.

When an NP requests a consultation from another provider, the N P should request “consultation” on the referral form, rather than “referring.”

Referrals:

A referral is made when the referring provider wants to turn the management of the patient over to the referred-to provider, at least for the current complaint.

When a NP refers a patient, the NP should state on the referral form that the NP is “referring the patient for evaluation and treatment.” The referred-to provider will bill an evaluation and management code, rather than a consultation code.

The APA code of ethics will absolutely assist me in reaching each stepping stone of my career goals in the HR world by providing most of the framework for my position. In my profession of HR especially, there are many rules and regulations which need to be met at all times. Ethical codes such as language, professionalism, value, confidentiality, unbiased, and record keeping are all standards which need to be upheld to be considered credible and reliable (Koocher & Keith-Spiegel, 2008). As I move closer and closer to my end goal, the more well versed I will be with many of these standards and will be able to teach and pass these ethical morals down to my other managers. I want my colleagues and coworkers to think of me as being fair, credible and accurate. Without the code, I don’t think I could reach the appropriate level of such behavior.

As a clinical psychologist and professor, Dr. Achim must uphold high ethical standards in his practice and in school, which I believe he does an excellent job of. Ethical standards such as confidentiality, responsibility, integrity, and respect. I think back to just last week with our discussions of multiple role relationships and that is probably a somewhat common occurrence in the clinical field when seeing a number of patients. Maintaining confidentiality with his patients along with maintaining professionalism and integrity with his students are two aspects of the code of ethics which help accredit Dr. Achim with such high standards and respect. As we can see, regardless of the profession I think the code of ethics can be related in so many ways.

Student two:

I would like to get my Ph.D. in psychology and become a clinical psychologist. To realize this goal, I will need to go through a process. First, I will need to get my M.S. in psychology from SNHU. I will then need to prepare for and take the psychology GRE test. Upon enrolling in a Ph.D. program, I will learn the skills that I need to succeed as a psychologist. I will spend a year of this time as an intern at a psychology clinic. After obtaining my Ph.D. in clinical psychology, I will spend another year as a post doctorate fellow at another clinic. I shall then look for employment. I will treat a variety of individuals, although I am particularly interested in adults with anxiety and depression.

The APA Code of Ethics will be most important to me when I become a clinical psychologist. In particular, I will rely on Section 3 of the Code of Ethics, which gives directives concerning harm, harassment, exploitation, and conflicts of interest (www.apa.org, 2017). I will need to treat my clients in a manner that will lead to healing. Healing is hindered by biases and distractions that stem from unethical behavior.

Dr. J (Dr. Achim) (You wanted me to pick on you): You have worked (and possibly still working) with children and the elderly. As such, you were/are bound by the standards of the APA. Hence, you must avoid exploiting the elderly and families of children. I will be bound in a similar manner when I become a clinical psychologist. For example, I could not overcharge an elderly lady for therapy when I become a clinical psychologist. When you worked performed forensic and assessment duties, you had to be honest. You were not allowed to ignore results that contradicted expectations and hopes. I will also be bound by the same expectations of honesty in the future. Finally, if a student asked you to go out for lunch, you would have to decline his or her offer; it could lead to future expectations and illusions of favoritism (I did this a long time ago with an older male student, but it ended well. It was not wise, however. He was a kind gentleman).

Research studies often compare variables, conditions, times, and/or groups of participants to evaluate relationships between variables or differences between groups or times. For example, if researchers are interested in knowing whether an intervention produces change in the desired direction, they will want to know whether the change is due to chance (statistical significance) or possibly due to the intervention. In this case, researchers could use a pre and post measurement of the same participants on the condition being treated, or they could compare a group of individuals who receive the intervention to a group that does not receive the intervention. Researchers could also compare two groups of individuals who receive different interventions. The rigor of the research design helps control for other factors that might account for the changes (e.g., time, conditions, group differences in other factors, etc.).
