Assessment Description

For this assignment, you will create a sustainability plan and present your plan using PowerPoint and Loom.

General Requirements:

A minimum of four scholarly or peer-reviewed research articles are required. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.
Solid academic writing is expected, and documentation of sources should be presented using APA formatting guidelines.
You are required to submit this assignment to LopesWrite for checking similarity scores.
Directions:

Based on the feedback you have received from your instructor on the “Sustainability Plan Worksheet,” (PLEASE SEE THE ATTACHED), compile the elements (measurement, ownership, communication, change management, and workload) into a presentation that you would present to the stakeholders of your proposed quality and/or safety sustainability program.

Develop a PowerPoint presentation (10-15 slides) to present your final sustainability plan. Create speaker notes of 100-250 words for each slide. For the presentation of your PowerPoint, use Loom to create a voice-over or a video. Refer to the topic Resources for additional guidance on recording your presentation with Loom. Include an additional slide for the title and Loom link at the beginning and an additional slide for References at the end.

Include the following:

Describe one or two measures that are critical to the sustainability of your proposed quality and/or safety sustainability program and decide on the data collection and monitoring process. Additionally, discuss whether you will reduce or eliminate the frequency of certain measurements.
Describe a measurement threshold that will trigger investigation.
Describe the owner of the improvement process who authorizes changes and will be responsible for sustaining improvement.
Outline a communication and training plan for improvements and learning. Consider who needs to know what, when and how often they need to know it, what communication method would work best, and who the best person is for initiating communication or sharing information.
Identify a change management theory that you will use to standardize the process so that it supports the improved process.
Explain how the proposed changes will affect staff workload. Ideally, the improvement has removed work and made people’s jobs easier and more efficient. Changes that decrease workload have a higher chance of success over time.

Develop a 4 page holistic intervention plan design to improve the quality of outcomes for your target population and setting.

Your application of the PICOT approach to developing your problem statement and the research that you conducted and synthesized in your literature review are the foundation and framework that you will need to successfully build your intervention plan. This plan will lay out specific components of the intervention you are planning to address the need you have identified for the target population and setting. You will justify your approach to the intervention plan by integrating appropriate theoretical foundations. You will also analyze and address the needs of stakeholders, requirements of regulatory bodies, and ethical and legal considerations. It is important to have a sound intervention plan design in place before trying to work on the details of implementation and evaluation.

Reminder: these instructions are an outline. Your heading for this this section should be Intervention Plan Components and not Part 1: Intervention Plan Components.

Part 1: Intervention Plan Components

· Define the major components of an intervention plan for a health promotion, quality improvement, prevention, education, or management need.

· Explain the impact of cultural needs and characteristics of a target population and setting on the development of intervention plan components.

Part 2: Theoretical Foundations

· Evaluate theoretical nursing models, strategies from other disciplines, and health care technologies relevant to an intervention plan.

· Justify the major componbents of an intervention by referencing relevant and contemporary evidence from the literature and best practices.

Part 3: Stakeholders, Policy, and Regulations

· Analyze the impact of stakeholder needs, health care policy, regulations, and governing bodies relevant to health care practice and specific components of an intervention plan.

Part 4: Ethical and Legal Implications

· Analyze relevant ethical and legal issues related to health care practice, organizational change, and specific components of an intervention plan.

Address Generally Throughout

· Communicate intervention plan in a professional way that helps the audience to understand the proposed intervention.

You are the NP working in the urgent care center. You have a 14 year old patient accompanied by his parents. The patients states a chief complaint of coughing for 2 weeks. The patient has a history of asthma which has been well controlled this past year. The patient is not currently on any daily medications. He is noted to also have morning sneezing and nasal congestion. It is reported that an increased number of students are sick with colds at school. He has no known allergies to foods or medications. He has history of eczema. How would you approach the medical management of this patient and why?
