There are a number of different options for determining how to charge for services provided at a health care organization. In the case-based service approach, one set fee covers an entire procedure. For example, if you needed knee replacement surgery, the fee would include preliminary visits, the surgery, and follow-up visits. However, in a fee-for-service approach, every different step would incur a separate cost. The reimbursement approach taken has significant ramifications for how care is provided.

To prepare:

  • Review the information focusing on reimbursement for services. How are these approaches the same or different in your own setting?
  • Identify an example of a fee-for-service approach and a case-based service approach.
  • Reflect on the benefits and limitations of each approach.
  • Review the article, “Which Health care Payment System is Best?” and examine the additional reimbursement strategies described. Consider the viability of these alternative strategies within your own organization (or one with which you are familiar).
  • Examine how the nurse-to-patient ratio is affected by the payment approach used and consider how this can impact quality of service.

    Measures of Crime and Causes

    Measuring crime can be a difficult process. By its very nature, crime is something that goes undetected. Law enforcement has developed a variety of techniques to track crime, such as police reports and victim reports. The Federal Bureau of Investigation (FBI) uses the Uniform Crime Reporting (UCR) Program for tracking crime; it reports crime in more than one way. All crime reporting and tracking systems categorize crime and have certain limitations.

    Measuring crime involves tracking statistics such as demographic information and moderator variables related to the crimes. Moderator variables are any third variable in a correlation that affects the relationship between the first two variables. For example, we may find that gender is related to violent crime with a higher percentage of males engaging in violent behavior. However, a moderator variable would be age, with the highest percentage of violent offenders being below the age of 30.

    It is also important for health care organizations to represent the communities that they serve in as many ways as possible. This diversity is represented in gender, race, ethnicity, age, and ability. Wynne (2012) posits that this diversity makes organizations stronger, allows for more productivity, and leads to enhanced job satisfaction. When health care organizations make themselves as diverse and inclusive as possible, they are integrating the community into the organization and the organization into the community, creating a win-win scenario for both. The health care organization is now more in touch with the needs, desires, and fears of the community. At the same time, the community members feel that they have representation in health agencies.
