Max Wellington is a 59 y/o male with a history of diabetes mellitus type two on oral meds, frequent urinary tract infections and hypertension. He presents to the office today with c/o of dribbling, frequency hesitancy and also now some erectile dysfunction which is more apparent due to the fact that he is in a new relationship.

Chief Complaint

“I am up 4-5 times at night having to pee. I am dribbling urine and sometimes I do not make it to the bathroom in time. It is also sometimes hard to get going once I get to the bathroom. I recently met a very nice woman and we are in a relationship, but I am finding it increasingly difficult to be intimate with her. Being up all night going to the bathroom and then going to the bathroom on myself is ruining my love life!”

Past Surgical History

Appendectomy age 14

Medications

Metformin 500mg po BID, Toprol XL 50mg po daily, Terazosin 10mg po daily.

Allergies

Seasonal allergies i.e. leaves and pollen, worse in the spring. No known drug or food allergies.

Denies heavy ETOH use, reports a glass of wine 1-2 times per week. Previous smoker, 1ppd for 20 years, quit in 2000. He is still working full time as a car salesman.

General

Denies recent fever, weight loss. Does admit to feeling fatigued because he is up so many times at night. Sitting comfortably in exam table, well nourished, in no acute distress.

Scenario 1: You have worked at your company for eleven (11) years. You have returned to college to earn a Bachelor’s degree in order to increase your chances for a promotion. You are nearly finished with your degree, when a supervisor’s position in a competing company becomes available in another state. The start date is in two (2) weeks, during your final exam period for your courses. The position offers a $15,000 per year salary increase, a car allowance, and relocation expenses. Scenario 2: Your child comes home from school with an assignment sheet for a school project. He / she is very excited about the project and begins work immediately, doing research on the Internet and gathering materials. You read over the assignment sheet and notice that your child is not including all of the required items in the project, and you have some ideas for how to improve the quality of the presentation. You recently read an article in a parenting magazine about the importance of a child developing responsibility for his/ her own learning. You recall the many ways in which your parents took over your school projects. You are coaching the soccer team, and you are in charge of fundraising for the band. You have a goal to complete your degree in two (2) years. Your doctor tells you that your blood pressure, your cholesterol, and your weight are too high and recommends several medications that cost you nearly $200 per month after your insurance co-pay.

Scenario 4: You are a sales representative for a company that encourages staff to log time in the field and away from the office. You are expected to begin and end your day at the office. You notice that each day when you arrive and return another co-worker is already there, and you wonder whether this person spends most of his / her time at the office. At your weekly sales meeting, you are informed of your co-workers’ outstanding sales performance. You suspect that this co-worker is spending more time flattering the boss instead of working leads in the field, and as a result is getting the best client referrals. Your own sales numbers have steadily decreased since this other sales representative was hired.

The notion of taking on “technical debt” during a project may seem perfectly natural to some stakeholders, but extremely controversial to others. Kent Beck emphasizes the value of evolutionary design highly in Extreme Programming. Technical debt can be said to be taken on whenever a less than optimal design is implemented in a product.

Matt Stephens and Doug Rosenberg vigorously attacked the idea of evolutionary design (among many other XP practices) in their 2003 book Extreme Programming Refactored: The Case Against XP. They argued that investing the time to create a robust product design is well worth the time, as it would eliminate the need to continually refactor an inferior product during its creation.

Understanding criminal law requires some knowledge about the sources of criminal law. These sources include constitutions, statutes, ordinances and regulations, common law, and the Model Penal Code. The U.S. Constitution, which is the highest form of law in our country, sets limits on the nature and extent of criminal law that the government can enact and serves as a guard of personal liberties. Common law still has an influence on today’s legal system. For instance, many state legislatures have a specific provision that recognizes the enforceability of common law through its courts, provided that the law is not inconsistent with the Constitution or laws of that state. In addition, the common law provides a persuasive authority in the interpretation of the criminal law. The Model Penal Code is a code of criminal laws intended to standardize general provisions of criminal liability, sentencing, defenses, and the definitions of specific crimes between and among the states. The Model Penal Code is not a law and has no binding effect. Rather, it is designed to serve as a model code of criminal law for all states.
