Consider a hypothetical scenario based on your own healthcare practice or organization that would require or benefit from the access/collection and application of data. Your scenario may involve a patient, staff, or management problem or gap. By Day 3 of Week 1

In the modern era, there are few professions that do not to some extent rely on data. Stockbrokers rely on market data to advise clients on financial matters. Meteorologists rely on weather data to forecast weather conditions, while realtors rely on data to advise on the purchase and sale of property. In these and other cases, data not only helps solve problems, but adds to the practitioner’s and the discipline’s body of knowledge.

Of course, the nursing profession also relies heavily on data. The field of nursing informatics aims to make sure nurses have access to the appropriate date to solve healthcare problems, make decisions in the interest of patients, and add to knowledge.

In this Discussion, you will consider a scenario that would benefit from access to data and how such access could facilitate both problem-solving and knowledge formation.

To Prepare:

  • Reflect on the concepts of informatics and knowledge work as presented in the Resources.
  • Consider a hypothetical scenario based on your own healthcare practice or organization that would require or benefit from the access/collection and application of data. Your scenario may involve a patient, staff, or management problem or gap.

By Day 3 of Week 1

Post a description of the focus of your scenario. Describe the data that could be used and how the data might be collected and accessed. What knowledge might be derived from that data? How would a nurse leader use clinical reasoning and judgment in the formation of knowledge from t

Discussion: Building a Health History

40-year-old black recent immigrant from Africa without health insurance

Post a summary of the interview and a description of the communication techniques you would use with your assigned patient. Explain why you would use these techniques. Identify the risk assessment instrument you selected, and justify why it would be applicable to the selected patient. Provide at least five targeted questions you would ask the patient.

Immigrating from one continent to another is a difficult task. A lot of people migrate for various reasons. People immigrate because of political, social, and economic reasons. As people move along, they encounter new problems and opportunities. It is essential for a person who has immigrated to another country to assimilate with the culture and the ways of doing things in their new environment. There are a lot of differences between countries in Africa and the United States when it comes to healthcare. A good example is that most African countries do not use health insurance systems; they all use pay as you go billing system. This means their entire healthcare bills are paid out of pocket.

To have a 40-year-old black man from Africa come for consultation without health insurance is difficult. I will like to ask the patient the following questions.

1. I will start by introducing myself to the patient.’

2. When was the last time he visited a doctor? The rationale is to know if we can get his medical history from his primary healthcare doctor

3. What is your family’s medical history? The rationale is to know what the patient is at risk for.

4. What is your medical history? This will enable the nurse practitioner to know what kind of sickness the patient has had in the past

5. The next conversation will be centered on how reliable is the patient’s mode of transportation. The rationale for this question is to see if the patient will be able to go for his medical appointment without any difficulties

6. I will also ask the patient what medication they are currently taking

7. I will also ask about their insurance situation and what method they will like to use for payment. The idea behind this question is to know if the patient will be able to pay his healthcare bill. Also, it is an opportunity to inform the patient about social services that can help the patients get health insurance. The patient will also be informed to contact social work for additional information.

Discussion: Pharmacokinetics and Pharmacodynamics

As an advanced practice nurse assisting physicians in the diagnosis and treatment of disorders, it is important to not only understand the impact of disorders on the body, but also the impact of drug treatments on the body. The relationships between drugs and the body can be described by pharmacokinetics and pharmacodynamics.

Pharmacokinetics describes what the body does to the drug through absorption, distribution, metabolism, and excretion, whereas p harmacodynamics describes what the drug does to the body.

When selecting drugs and determining dosages for patients, it is essential to consider individual patient factors that might impact the patient’s pharmacokinetic and pharmacodynamic processes. These patient factors include genetics, gender, ethnicity, age, behavior (i.e., diet, nutrition, smoking, alcohol, illicit drug abuse), and/or pathophysiological changes due to disease.

For this Discussion, you reflect on a case from your past clinical experiences and consider how a patient’s pharmacokinetic and pharmacodynamic processes may alter his or her response to a drug.

To Prepare

· Review the Resources for this module and consider the principles of pharmacokinetics and pharmacodynamics.

· Reflect on your experiences, observations, and/or clinical practices from the last 5 years and think abo out how pharmacokinetic and pharmacodynamic factors altered his or her anticipated response to a drug.

· Consider factors that might have influenced the patient’s pharmacokinetic and pharmacodynamic processes, such as genetics (including pharmacogenetics), gender, ethnicity, age, behavior, and/or possible pathophysiological changes due to disease.

· Think about a personalized plan of care based on these influencing factors and patient history in your case study.

By Day 3 of Week 1

Post a description of the patient case from your experiences, observations, and/or clinical practice from the last 5 years. Then, describe factors that might have influenced pharmacokinetic and pharmacodynamic processes of the patient you identified. Finally, explain details of the personalized plan of care that you would develop based on influencing factors and patient history in your case. Be specific and provide example
