Section 1: Organizational Systems and Structures Evaluation

When a problem related to health care quality and patient safety arises, there can be an

inclination to focus on a specific event rather than to examine the larger context

surrounding it. Analyzing the systems and structures within an organization provides a

foundation for generating a fuller understanding of how and why the event occurred and

for developing strategies to address the underlying issues. Perhaps more importantly,

such an analysis can be used to proactively identify potential challenges and improve

organizational systems and structures in order to promote positive outcomes.

In the first section of your Course Project, you analyze the systems and structures of a

health care organization with which you are familiar.

To prepare:

Identify a health care organization with which you are familiar; one that you will be

able to sufficiently analyze to complete the full scope of the Course Project. (Refer to

the Course Project Overview as needed.)

Select two or more of the following frameworks:

· Learning organizations

· Complex adaptive systems (CAS)

· Clinical microsystems

· Good to great

· The 5 Ps

With these frameworks in mind, analyze the systems and structures of the

organization. Be sure to research and consider:

· Mission, vision, and values

· Strategic plan, goals, and objectives, if possible

· Key operational processes and patterns

· Information technology use

· Organizational priorities and investments, as indicated by financial data

Week 4: Organizational Systems and Structures, Part 2

Section 1: Organizational Systems and Structures Evaluation

As you continue your evaluation of a health care organization, it is essential to pay

attention to culture. Organizational culture provides the context in which all interactions

and processes occur and is therefore central to any effort to enact change.

To prepare:

Continue to analyze your selected organization, as indicated in Week 3 (Section 1 of

the Course Project).

Continue to analyze essential elements of organizational culture and evaluate the

influence of culture on the ability to achieve goals within your selected organization.

To complete:

Write a 3- to 5-page paper in which you:

· Provide a description of the organization that you selected

· Present your analysis of the organization with attention to:

· Its mission, vision, values

· Strategic plan, goals, and objectives

· Key operational processes and patterns

· Information technology use

· Organizational priorities and investments, as indicated by financial data

· The essential elements of the organization’s culture

· The influence of culture on meeting organizational goals

Section 2: Quality Improvement Issue

To help you move forward with your Course Project, this week you identify a quality

issue that you would like to address and submit a description of this issue for Instructor

feedback.

Your proposed issue should reflect a genuine need for improvement within your

selected health care organization. Through your Course Project, you will analyze this 

issue and propose strategies to improve outcome(s), including the redesign of a related

process.

To prepare:

· Review the six quality improvement aims from the Institute of Medicine (IOM)

· Safe: Avoiding harm to patients from the care that is intended to help them.

· Effective: Providing services based on scientific knowledge to all who could benefit and refraining from providing services to those not likely to benefit (avoiding underuse and misuse, respectively).

· Patient-centered: Providing care that is respectful of and responsive to individual patient preferences, needs, and values and ensuring that patient values guide all clinical decisions.

· Timely: Reducing waits and sometimes harmful delays for both those who receive and those who give care.

· Efficient: Avoiding waste, including waste of equipment, supplies, ideas, and energy.

· Equitable: Providing care that does not vary in quality because of personal characteristics such as gender, ethnicity, geographic location, and socioeconomic status.

· Conduct a search of the literature to help you identify a specific issue that warrants attention and action to promote quality improvement.

· Consider what you have surmised thus far through your analysis of the organization

that you selected. What have you noticed that could help you identify a quality related need for improvement? For instance:

· What has captured your attention in meetings, reports, and/or daily activities?

Cancer, Genetics, and Genomics

Rachel comes from a family with a history of breast cancer on her mother’s side. Rachel’s mother died of breast cancer when she was very young. Rachel has two sisters, Lisa and Kristin. Rachel has remained close to Lisa, but she no longer has a relationship with Kristin. At a routine checkup, Rachel is told about the availability of genetic testing for identifying a predisposition to breast cancer. Her doctor recommends the test to Rachel given her family history. Rachel has the genetic testing done and finds that she has a mutated breast cancer 1, early onset (BRCA1) gene. Her doctor tells her she is at high risk for developing breast and ovarian cancer. Rachel’s doctor suggests she ask her sisters to be tested also, so they can take the proper preventative measures. Rachel feels comfortable sharing this information with Lisa, but she has not spoken to Kristin in many years. Rachel tells her doctor that she is not in contact with Kristin and will not make an effort to tell her about BRCA1 and genetic testing. Rachel’s doctor feels confident that she can locate Kristin but worries about breaching patient confidentiality if she goes against Rachel’s wishes.

Cultural awareness and sensitivity has become a necessary tool in nursing because; given the diversity in the growth of different populations, it has become imperative for nurses to understand various cultures and also respect the different practices in order to ensure holistic patient care at all levels of management. One among other tools that have been developed to help gather cultural information is the Heritage Assessment Tool. Through the outcomes of the survey from this tool, a nurse is able to gather relevant information about a patient’s culture, beliefs, needs and practices that may affect their care and treatment. While the tool is useful in structuring individualized patient care plans, it is also helpful in educating nurses adequately on new cultures thereby expanding their cultural competency foundation (Falkner, 2018). Some of the other tools used include; CCCC model, ACCESS model, The Process of Cultural Competence in the Delivery of Healthcare Services Model among others (Higginbottom, Richter & Mollel, 2011). Regardless of the tool being used the relevance remains in how the data would be used in the caring for patients.

Cultural competency is the act of being aware of different cultural practices that helps a nurse to adjust care plans appropriately and to constantly remain sensitive and respectful of the choices that patients make based on those practices. Cultural competency leans on cultural awareness through thorough assessment of patients and its relevance to patient care is emphasized on ensuring safety at all costs. The nurse therefore has the responsibility to understand that some cultural beliefs have health practices that are considered unsafe to other cultures that don’t share in the same beliefs. For example, the [practice of female genital mutilation seen in middle east countries, Africa and southeast Asia is a practice where the female genitalia is partially or totally removed for non-medical purposes (WHO, 2018). Because of the many risks this practice poses on women healthy, the nurse caring for such clients must remain sensitive especially in their choice of words. Furthermore, child protection issues should be taken into account and advocacy for [patient safety should be considered if a minor is to undergo this procedure. Hence, understanding a client’s cultural preferences if beneficial for effective patient care (Falkner, 2018). Even though various barriers to health exist among different cultures, nurses are to remain focused on enhancing quality patient care by ensuring safety.

Some of the ways cultural competency can be demonstrated is first, through comprehensive assessment. It is from such gained information that the nurse is able to understand family dynamics from which a customized care plan can be formulated to accommodate the needs. Secondly, while addressing emerging populations of immigrants, the nurse should be aware that most individuals lack knowledge to navigate the complex healthcare system and thus through collaboration with other interdisciplinary members, interpretive services should be provided to ensure effective management and smooth transition of care. Lastly, through cultural competency, nurses are able to identify vulnerable populations such as pregnant women, human fetuses, cognitively impaired et cetera, and by working with other interdisciplinary members, that helps to ensure provision of resources that would enhance adequate health care.

A nurse gathers cultural information from patients with care that is self- reflecting and an open mind. Itsimportant for the nurse to remain sensitive and respectful as the goal of the therapeutic relationship is to eliminate health disparities. “A nurse must give high quality care regardless of ethnicity, social economic background, race or culture” (ANA )

Cultural competence is an evolving process and is a daily process. Our communities are a mix of Asians, African Americans/ blacks, Hispanics, Arabs who all deserve respect and culturally sensitive care. To be Culturally competent, it “requires continuous education, self-awareness, and evaluation in order to provide holistic, culturally competent nursing care” (Campinha-Bacote, 2011).

Prior to this material review I was not aware such a framework existed for a cultural assessment. Leininger states that cultural assessment “is the systematic appraisal or examination of individuals, groups, and communities as to their cultural beliefs, values and practices to determine explicit needs and intervention practices within the context of the people being served” (Falkner, 2018). The Heritage Assessment tool is a great resource for helping the nurse determine cultural influences in managing cultural care focus in education. It surprised me that I was using it all along, mostly for my own exploration/ self-reflection. I am a very cultural curious nurse and learning about cultures helps navigate my care. 

Culturally competent care has been linked to improved health outcomes and the elimination of health disparities. To demonstrate competency is to emerge yourself in it. Learn from them. Most importantly nurses must have an “acknowledgement of culture as a core nursing trait of caring” (Gillson & Cherian, 2019). I am culturallycompetent, and I ask these questions regularly: 

Where you born?

Where did you grow up?

How old were you when you moved to the US?

Tell me how religion is important to you? 

Tell me about the foods you eat?

How do you celebrate holidays, deaths?

Module 03 Written Assignment – Cultural Considerations in Mental Health Nursing

Submit an evidence-based practice paper about cultural competency in mental health nursing. An evidence-based practice paper allows you to explore best practices and help improve client outcomes in a psychiatric unit. Your paper should describe how you, as a nurse, will include – or have included – cultural awareness in a client diagnosed with a mental illness. The person you describe could be someone in your clinical setting, someone you have worked with in the past, or a theoretical client.

Remember that culture can also include gender equality, sexual orientation, and other cultures besides race and religion – including cultures unique to one particular family.  Your paper should be at least 3 pages (double-spaced, not including the title or reference pages) in APA format and include:

Assessment: Discuss what you would assess in regard to a client’s culture. For example, are there specific dietary requirements? Are schedule changes necessary to avoid conflicts with religious practices? Who is the spokesperson for the family? What would you assess?

Diagnosis: List any mental health nursing diagnoses this person has or may be at risk for. Include at least one cultural diagnosis.

Planning: What planning needs to be done to ensure the cultural and emotional safety of the client?

Implementation: What are interventions that would ensure the safety of your client in regard to culture? Include at least two interventions. For example, if your client has religious beliefs that affect the ability to take medications, what interventions would you create to ensure the client’s safety? What can you do to make sure the client’s cultural needs are met?

Evaluation: How will you evaluate whether your implementation was effective? Make sure the parameters are objective and measurable.

In your summary, discuss whether any completed interventions were successful. What could be done differently in the future? If the interventions have not yet been carried out, you might discuss some institutional changes that could be made to ensure cultural safety for all clients in that setting.
