Tomas, Kindergarten

Tomas entered Mrs. Richards’ Kindergarten classroom at the beginning of the school year with great excitement! He showed great interest in learning and being in the classroom with other students. As the school year progressed, however, Tomas’ excitement quickly turned into frustration. He struggled with recognizing the letters in his name, identifying different shapes, and consistently could not follow two-part instructions. He also was dramatically less able than his peers to focus on a task. His frustrations have led to impulsive actions. Mrs. Richards has called a meeting with his parents to address her concerns.

Harper, 3rd grade

Harper attends Sunset Elementary School and has been placed in a class with 25 other 3rd graders. Harper loves her teacher Mrs. Hernandez and struggles when a substitute takes her place. She excels in math and tends to get bored when the other kids in her class struggle. Harper also loves reading about the weather, somewhat obsessively, and can share weather facts and details for hours. She enjoys going to school, but does struggle with the loud noises a school brings. In school assemblies, for instance, she becomes overly upset about the noise level and tends to rock back and forth to calm herself.

 

Austin, 5th grade

Austin was 9 years old when he was hit by a car while riding his bike on the sidewalk. He broke his arm and leg and also hit his head very hard. When he came home from the hospital he looked just fine, but he needed lots of help. Now back at school, there are changes in Austin that are hard to understand. It takes Austin longer to do things and he has trouble remembering. He can’t always find the words he wants to use. Math is hard for him now, but it was his strongest academic area before the accident.

    With health care reform two objectives were being addressed 1) being separate Medicaid from welfare making easier to qualify for Medicaid and the second is welfare policies being more restrictive to those applying (Kronebusch n.d). The basic model for Medicaid enrollment include the characteristics of the family and is based on certain qualifications such as income, family size, head of household and if the family is US citizens or not. The variables included the family’s size and how much was being brought into the home in order to qualify. The evolution of health care policy have impacted many families throughout the years as the range of income status continues to change. With the goal intended to separate welfare and Medicaid it appears more families are being offered healthcare and receiving the necessary health care coverage needed.

    In the state of Nevada our current health care policy based on Medicaid is that the families have option for three choices, two in which qualify families for HMO Medicaid (Amerigroup & health plan of Nevada) based on families choice. With each HMO the family is provided a case worker to ensure the family needs are being met. The policy I would amended are the qualifications for Health Plan of Nevada as it is rarely accepted throughout the state and the families are paying a certain monthly fee but not being provided the appropriate care. I would advocate that more providers qualify to accept Health plan of Nevada and look into eligibility requirements. The stakeholders involved within that decision is the Department of Health care and financing and the Nevada Medicaid task force (NDHHS n.d).

    Health care programs and policies have evolved since 1965 when legislation established Medicare and Medicaid. When Medicare and Medicaid was first established it declared that the federal government wouldn’t interfere with clinical medicine (Dewalt, et al 2005). Since then the evolution of medical care, its finances, and clinical medicine has been linked to Medicare (Dewalt, et al 2005). Congress involvement with Medicare and Medicaid begun when concerns with fraud and abuse of the system became evident. Congress still has control over Medicare and Medicaid and it continues to evolve with the Medicaid health reform act or Obama care.

    Michigan has a policy called processing request 10.2 A states that a verbal prior authorization may be requested over the phone. This is only for situations that require amidated correction or diagnosis to prevent further damage. If it is after hour the provider may submit the request for authorization the following day (Michigan Medicaid manual, 2016). The change I would make to this policy is not requiring prior authorization for diagnosing or treating and issue that would create damage to a person. I feel that this could cause a delay in a lifesaving process.

    M4 Discussion 2: Research and Theory

    The clinical aspect of human services involves a variety of strategies and practices, all of which involve ethical principles and considerations. These do not come out of thin air, however; they are based on research findings and grounded in theory. To be successful in this field, it is important to understand this research and these theories, and how they connect with specific practices.

    The first thing to note about how the British North Americans and Spaniards of the New World saw race is that their societies had racial hierarchies (Burnett et al., 2018). The Spanish “Sistema de Castas” divided people into racial groups based on their “blood purity” (Burnett et al., 2018). Peninsulares, Spaniards born in Ibera, were at the top, while slaves and “Indios” were at the bottom (Burnett et al., 2018). Mixed raced “mestizos” lacked the privileges of full-blooded Spaniards, but weren’t as disadvantaged as full-blooded Indios (Burnett et al., 2018).

    Meanwhile, in British North America, colonists used Africans and Native Americans as slaves (Burnett et al., 2018). They took more and more of their rights away. In 1643, a law was passed in Virginia that made African women “tithable”, meaning they had to do agricultural labor (Burnett et al., 2018). This was done to separate them from white women, who didn’t have to do manual labor (Burnett et al., 2018). Later, in 1662, a Virginian law stated that all children born of an enslaved woman would be a slave for life, even if the father was white or free (Burnett et al., 2018).

    The second thing to note about how these Europeans viewed race is that they initially did not see race the same way they later would. Most Europeans would not racially identify with Europeans from other countries (Burnett et al., 2018). The idea that race involves an inherited physical difference which categorized Europeans together and separated them from Africans, Asians, and other races, was new (Burnett et al., 2018). It was born from the Atlantic slave trade, and was used to support and justify oppression (Burnett et al., 2018).

    Nowadays, people aren’t so strictly divided into racial hierarchies. There are people of all races who have become rich and enjoy a life of comfort. Even people who are born into disadvantageous situations can potentially rise to the top. At the same time, racism does unfortunately still exist. There are many black people who, while not technically kept down by the law, are trapped in lives of poverty. We also still keep that same idea of race as “inherited physical differences that group Europeans, Africans, and so on separately” (Burnett et al., 2018). While people will of course recognize the difference between a Frenchman and an Englishman, many still see these different kinds of people as “white”, and thus belonging to the same group. Many people even have trouble telling the difference between non-Europeans of different cultures and nationalities, instead opting to lump them all together as one big racial group.
